Medicinal Mushrooms & the Immune System: What Does the Science Really Show?

Medicinal mushrooms have attracted increasing scientific interest because they contain biologically active compounds that may interact with the immune system. Among the most studied are β-glucans and other polysaccharides, which researchers are investigating for their potential effects on immune-cell activity and signaling. But there is an important distinction between biological activity, laboratory findings, and proven clinical benefits. So, what does the science actually tell us?

How Might Medicinal Mushrooms Interact With Immune Cells?

The immune system constantly monitors the body for signals associated with infection, tissue damage, and abnormal cells. Certain mushroom-derived β-glucans can be recognized by receptors involved in innate immune surveillance. These receptors are present on immune cells such as:

  • Macrophages
  • Dendritic cells
  • Neutrophils and other innate immune cells

When β-glucans interact with these receptors, they can influence intracellular signaling and immune-cell behavior. Preclinical research suggests that these interactions may affect processes such as immune-cell activation, cytokine signaling, and communication between components of the immune system.

However, an important scientific principle applies here: a measurable immune response does not automatically translate into a health or treatment benefit in humans.

From Immune Modulation to Cancer Research

The relationship between the immune system and cancer is an active area of research. Cancer cells can interact with surrounding immune cells and the tumor microenvironment in complex ways. Researchers are therefore interested in compounds that may influence immune responses against abnormal cells.

Studies involving mushroom-derived compounds have investigated effects involving:

  • Natural killer (NK) cells
  • T cells
  • Macrophages
  • Dendritic cells

These findings provide a biological rationale for investigating medicinal mushrooms and their compounds as potential adjunctive or supportive approaches in cancer care. However, laboratory or animal findings cannot be automatically interpreted as evidence that a mushroom supplement can prevent, control, or cure cancer. The key question is whether these biological effects translate into meaningful improvements in patients.

What Do Human Studies Show?

Human evidence is considerably more complicated. A systematic review examining clinical trials of fungal β-glucans in cancer patients identified 16 trials involving 1,650 patients. Some studies reported potentially interesting effects, including changes associated with treatment-related immune suppression or recovery of white blood cells.

However, the evidence was not uniform. The studies differed in:

  • Mushroom species
  • β-glucan composition
  • Extraction and preparation methods
  • Dosage and treatment schedules
  • Cancer types
  • Conventional treatments being used
  • Clinical outcomes being measured

Because of these differences, results cannot simply be combined into the conclusion that “medicinal mushrooms boost immunity” or “fight cancer.”
